Sports injury
Hamstring strains, ankle sprains, rotator cuff and runner's knee. We test the movement that hurt you, not just the joint that aches.

Outpatient physical therapy · Portland, OR · Est. 2014
Every patient at Baseline starts with a 60-minute evaluation, leaves with a written plan of care, and sees their own range-of-motion numbers move week over week.
No 15-minute handoffs and no rotating cast. You work with the same licensed physical therapist from your first visit to your last.
Perfect for: runners and lifters coming back from injury, post-op knee and shoulder patients, and anyone tired of pain they've been told to live with.
What we treat
We are a small clinic on purpose. Four licensed physical therapists, six treatment rooms and a 1,900 sq ft movement floor — enough to cover the cases we're genuinely good at, and not one more.
Hamstring strains, ankle sprains, rotator cuff and runner's knee. We test the movement that hurt you, not just the joint that aches.
ACL, meniscus, rotator cuff, total knee and total hip. We work from your surgeon's protocol and send progress notes back after each phase.
Low back, neck and widespread pain lasting longer than three months. Graded exposure, education and a load plan you can actually keep to.
Pre- and postpartum care, pelvic pain and continence. Private room, gowning available, and a therapist with dedicated pelvic-floor training.
Fall prevention, vestibular rehab and strength for adults over 65. Twice-weekly small-group classes run alongside one-to-one care.
Hop testing, limb symmetry index and strength ratios before you're cleared. If the numbers aren't there, we say so and keep working.
How care works
Physical therapy goes wrong when nobody writes down where you started. We measure on day one and re-measure every third visit, so progress is a number rather than an opinion.
01 — EVALUATION
Movement screen, strength and range-of-motion baseline, and a conversation about what you actually need to get back to. You leave the first visit with exercises, not a follow-up booking slip.
02 — PLAN OF CARE
Your therapist writes the number of visits, the milestones for each phase and what we'll stop doing if a phase isn't working. Your physician or surgeon gets a copy if you want one.
03 — PROGRESS & DISCHARGE
Same measurements, same therapist, printed side by side. When your numbers hit the discharge criteria we wrote in week one, we discharge you — with a maintenance program, not an open-ended plan.
The clinic
Baseline was started in 2014 by Marguerite Hale, DPT, after eleven years in a hospital outpatient department where therapists routinely ran three patients at once. The whole clinic is built to make that impossible.
Our method
Manual therapy buys you a window where the joint moves better and hurts less. What you do inside that window is what actually changes the tissue — so every session ends with load, not a heat pack.
That means the second half of your hour is usually the least comfortable and the most useful. We tell patients this on day one so it isn't a surprise in week two.
The techniques we use, and whenYour therapists
Every therapist at Baseline is a licensed physical therapist practising in Oregon, and each carries a caseload capped at nine patients a day so nobody gets rushed through.
Founder · Post-surgical & knee
Eleven years in hospital outpatient care before opening Baseline in 2014. Runs the ACL return-to-sport testing protocol.
Pelvic health & perinatal
Board-certified in women's health physical therapy. Sees pre- and postpartum patients in the private room off the east corridor.
Sports rehab & running
Former collegiate distance runner. Handles gait retraining, tendon loading and the Tuesday-evening runners' clinic.
Patient outcomes
Three patients discharged in the last eighteen months, with their permission and their own words. Names shortened at their request; the numbers come from their charts.
"I'd been told my back was 'just wear and tear' for six years. Tomas gave me a number on day one and a different number on week seven. Nobody had ever shown me the difference before."
Chronic low back pain · 12 sessions
Dana R. Warehouse supervisor, Gresham
"My surgeon cleared me at four months. Marguerite ran the hop tests and told me my left leg was still 22% weaker. She was right, and I'd have torn it again."
ACL reconstruction · 21 sessions
Kieran M. Amateur footballer, SE Portland
"Six weeks postpartum I couldn't sneeze without leaking and I'd been told to wait it out. Adaeze had me running again by August. I wish I'd come in a year earlier."
Postpartum pelvic health · 9 sessions
Priya S. Teacher, Beaverton
Rates & billing
Our cash rates are below. If you're using insurance we'll verify your benefits before your first visit and tell you what your visit will cost — in writing, before you're charged.
$185 60 MIN · ONE-TIME
The full assessment: history, movement screen, strength and range-of-motion baseline, and your first set of exercises.
$140 60 MIN · PER VISIT

$780 SAVE $60 · PREPAID

Insurance. We bill most major commercial insurers as an in-network or out-of-network provider, and we are a Medicare-enrolled practice. Coverage varies by plan, so we run a benefits check before your first visit and quote you your expected per-visit cost in writing.
If you're paying cash. The rates above are the rates — there is no separate facility fee, no charge for equipment, and no additional billing for the exercises you take home. Cancellations inside 24 hours are charged at $60.
Before you book
Still unsure? Call the front desk on (503) 555-0148 and ask for a five-minute triage call with a therapist. There's no charge for it.
Not to start. Oregon has direct access, so you can book an evaluation with us without seeing a physician first. Some insurance plans still require a referral for reimbursement — we check that for you during the benefits verification, before you're billed for anything.
Most patients notice a change in symptoms within three to four visits, and our average course of care runs 8.4 sessions. Post-surgical rehab is longer by nature — an ACL reconstruction is typically 12 to 24 visits across nine months, in phases we map out at the start.
Clothes you can move and be examined in — shorts for anything below the waist, a vest or loose t-shirt for shoulders and necks. Bring your insurance card, a photo ID, any imaging reports you have, and the shoes you actually train or work in. We have gowns and a private changing room if you'd prefer them.
Yes. You're assigned to one therapist for your entire plan of care, and we hold their schedule so your appointments land at the same time each week where possible. If they're away, we'd rather move your session than hand you to someone who hasn't read your chart.
Most of our caseload isn't athletes. Roughly 60% of the people we see are working adults with back, neck or shoulder pain, or adults over 65 working on balance and strength. "Performance" in our name means getting back to whatever you were doing before — a shift on your feet counts.
We'll tell you and refer you on. Around one in fourteen evaluations ends with us sending someone to a physician, an orthopaedic specialist or a different discipline instead of booking follow-ups. You're charged for the evaluation and nothing further.
Book an appointment
Evaluations are booked within five working days, and there are usually two same-week slots held back for acute injuries. Tell us what happened and we'll find you an hour.
