Your first visit
What to expect and bring
- Arrive ten minutes early — there's paperwork on a first visit
- Bring your insurance card, photo ID and any imaging reports
- Wear clothes you can move and be examined in
- Bring the shoes you actually train or work in
- The evaluation runs a full 60 minutes; you'll leave with exercises
- Gowns and a private changing room are available on request
Insurance & billing

We'd rather have the awkward conversation now than on an invoice later. If your question isn't here, call (503) 555-0148 and ask for billing.
We bill most major commercial insurers, as an in-network or out-of-network provider depending on the plan, and we are a Medicare-enrolled practice. Rather than publish a list that goes stale, we run a benefits check on your specific plan before your first visit and tell you what you'll owe per visit — in writing.
Self-pay rates are published: $185 for the initial evaluation, $140 per follow-up session, or $780 for a prepaid six-session package ($130 per session). If you're using insurance your cost depends on your deductible, copay and coinsurance — which is exactly what the benefits check tells us before you commit to anything.
Sometimes, particularly if you have a high deductible you're nowhere near meeting. The benefits check gives you both numbers side by side and you choose. We don't get paid more either way, and we'll tell you plainly which one comes out lower for your plan.
Yes, for accepted Oregon claims. Bring your claim number and your adjuster's contact details to the first visit. We bill the carrier directly and send them progress reports at the intervals they require, at no cost to you.
Cancellations inside 24 hours are charged at $60, and we waive that once per plan of care because life happens. Because every session is a full hour with one therapist, a late cancellation is an hour nobody else could book — which is the whole reason the policy exists.
They're refunded at the package rate when you're discharged. If you finish in four sessions instead of six, you get the difference back — we'd rather that than have you booking visits you no longer need. Packages are valid six months and transferable within a household.
